      Please call us at (440) 235-8099 to discuss your vision plan and medical plan prior to your scheduled appointment. This will allow us to have the proper prior authorizations in place and best inform you of what your contribution will be at the time of your office visit.

      Regardless of whether you have nsurance coverage or not, we have very affordable prices and provide the best value to patients paying in full for their own eye care and vision needs. We are often able to give a discount to cash paying patients and appreciate you choosing us when there are many options available to you. 

      We are committed to providing you with the best care and ophthalmic products. If you have vision coverage for the services provided, we will help you maximize your benefits. In order to achieve these goals, we need your assistance and understanding of our payment policy. You must realize that your insurance is a contract between you and your insurance company. Not all services are a covered benefit in all contracts. Some insurance companies arbitrarily select certain services they will not reimburse.

      We often have patients that have both VISION and MEDICAL insurance. They are very different in terms of the services they cover and it’s important for our patients to understand those differences. VISION coverage is mainly designed to determine a prescription for glasses, help pay for eyeglasses or contact lenses, and to evaluate the general health of the eyes. It is not designed or equipped to deal with MEDICAL conditions, diagnoses, and/or treatment plans or necessary follow-up visits.

     When a MEDICAL diagnosis or condition is present (such as high blood pressure, diabetes, or an eye disease such as infections, dry eyes, allergy, and cataracts, to name just a few) it is necessary to file the claim for your visit with your major medical carrier and the co-pays for that insurance will apply as well as any non-covered service like the refraction, or finding of your prescription. VISION insurance does not cover medical eye problems, just as MEDICAL insurance does not cover routine vision testing. Our office does not make these rules; they are defined by the insurance carriers themselves.

      There is no way to know prior to the examination which type of insurance will apply or with whom our office will be able to file a claim for you. We make every effort to be a provider on every insurance carrier for your convenience and we will file those claims for you when there is a medical problem. In the event that we are not a provider for your medical/vision insurance, we will provide you with an itemized receipt so that you may file with your carrier and get the reimbursement sent to you.

     The following payment plans have been established for our patients.

          1. We accept cash, personal check, debit card, Visa, Mastercard, Discover credit cards and Care Credit. 

           2. All materials must be paid for in full at the time order is placed.

     Please bring your current insurance card with you to the office on your first visit and notify us of any changes when they occur. Please remember that no insurance company attempts to cover all costs. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any co-pays, deductible amount, co-insurance, or any other balance not paid for by your insurance company. Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment.
